Terms of Business

  1. I agree to entrust the booked in patient for investigation, diagnostic procedures, and such medical or surgical treatment as the veterinary surgeon dealing with the case considers necessary. 

  2. As a standing sedated procedure is to be administered, there are risks associated with standing anaesthesia. I understand and accept the attendant risks which have been clearly explained to me. 

  3. Where nerve block procedures are administered, there are risks associated. I understand and accept the attendant risks which have been clearly explained to me. Such risks include damage to the eye and surrounding structures.

  4. Where the horse is to be left in our care (at day clinics or overnight) I accept full risks and liabilities for any injury or complications encountered.  I give consent for my horse to be cared for and treated as necessary during this time.  Equine Veterinary dentist Ltd will not be held liable for any accident caused during this period of hospitalisation.

  5. I accept there may be certain risks to the life of the horse, which could occur as a result of the proposed treatment. These risks have been fully explained to me. I appreciate that whilst reasonable care and attention will be given to the horse, no member of staff shall be liable for loss, damage or injury resulting from treatment or otherwise. 

  6. I understand that certain useful drugs, which are not licensed for use in horses, may be administered at the discretion of the veterinary surgeon, and I fully accept this and any attendant risks. 

  7. I agree to be invoiced direct by Equine Veterinary Dentist Ltd, and I agree to pay to the Equine Veterinary Dentist Ltd all reasonable costs and expenses that may be incurred whilst the horse is under its care. I understand that if complications occur, costs may be in excess of any first quoted. I agree to pay all balances in full.

  8. Should it prove necessary, in the opinion of the attending veterinary surgeon, I agree to the destruction of the horse on humane grounds. I understand that, should humane destruction be considered necessary, further written authorisation will be requested from me. 

  9. I agree that all case papers, radiographs, video recordings and materials of all kinds remain the property of the Equine Veterinary Dentist. 

  10. In the case of insured horses, I acknowledge that my insurance company has been informed that the horse has been referred for diagnosis, and/or treatment.  I also accept that any costs not covered by the insurance company will be my responsibility to pay 

  11. To the best of my knowledge I certify that this horse will not enter the human food chain. 

  12. I agree that I understand the estimated costings for common treatments.

  13. Equine Veterinary Dentist Ltd does not accept any liability or responsibility for any personal injury or accident that occurs during any treatments.

  14. Equine Veterinary Dentist Ltd does not accept any liability or responsibility for any damage or injury to any animals or personal property that occurs during treatment.

  15. All clients must have In place emergency out of hour cover from their registered veterinary practice for all animals treated by Equine Veterinary Dentist Ltd.  This should cover any unlikely emergency complications arising in the hours to days following any dental procedures.  Equine Veterinary Dentist Ltd have a 24h advice service and reassessment appointments where they are required.

  16. In cases where appointments are cancelled with less than 48h notice to ourselves - a cancellation fee of £80 plus Vat is applied.  This must be paid within 7 days from the date of invoicing.

  17. A fee of £20 plus Vat is charged for the completion of all administration related to insurance claims.

  18. All clients that book at appointment with Equine Veterinary Dentist Ltd agree to the above terms of business and are responsible to fully read the terms in their own time.
